Peterborough College is seeking a dynamic and dedicated Transitions In & Out Officer to join our vibrant and supportive team. Key Responsibilities include:

Managing a caseload of students, providing Information, Advice, and Guidance, and developing bespoke transition plans.

Attending EHCP Annual Reviews and presenting tailored transition plans.

Creating and delivering study programmes that focus on practical, real-world transition activities.

Connecting students with essential post-college providers and facilitating supported transition periods.

Supporting students to access external agencies and maintain social connections.

Organising events for students and their families to inform them about successful transition planning.

Promoting equality, diversity, inclusion, and safeguarding practices.
