At Stamford College, we’re looking for a compassionate, organised and proactive Transitions In & Out Officer to support students with moderate to complex needs as they prepare to leave full-time education and move into the next exciting stage of their lives.

About the Role

You’ll work closely with our SEND and Inclusive Learning teams, external agencies and families to develop and deliver transition plans that focus on the four key areas of Preparing for Adulthood:

  • Employment
  • Independent Living
  • Community Inclusion
  • Good Health

From attending EHCP reviews and coordinating visits to post-education providers, to helping students maintain friendships and access support services, your work will play a vital part in ensuring no learner is left behind.

What We’re Looking For

  • Someone with experience supporting young people and developing tailored plans
  • A clear communicator and advocate, especially in multi-agency settings
  • Passionate about inclusivity, with knowledge of the SEND Code of Practice
  • Confident using Microsoft Office and managing caseloads effectively
  • Positive, patient and genuinely motivated to empower learners
