Education Health Care Plan / Transitions Assistant. Full Time. Permanent.
In this role you will assist in the delivery of the college’s statutory duty to review Education Health and Care Plans (EHCPs) of young people with Special Educational Needs and disabilities. Working in partnership with Additional Learning Support Coordinators, you will organise annual reviews, prepare reports and effectively deliver the College obligations in relation to the annual review of EHCP’s across the academic year in partnership with families, schools and other professionals. You will also develop transition plans and events to enable students to make a smooth transition from school to college.
Excellent communication skills both written and verbal with the ability to deal sensitively with students and their families are essential. You will also be a confident user of IT systems and be able to maintain accurate records and work with minimal supervision. Experience of working with people who have additional needs would be an advantage.
You will be educated to Level 3 standard and will have achieved a Level 2 or equivalent qualification in maths and English, together with a Level 2 in IT or have relevant experience with Microsoft packages.
