An exciting opportunity has arisen in the Disability Inclusion Team working with children and young people from birth to nineteen years old.
We are looking to appoint a Senior Transition Worker as part of the Education Service and with partners in many different agencies.
The role focuses on ensuring that the needs of learners with significant and/or complex medical and physical disabilities are supported to access their entitlement, have their needs met and are able to make good progress within the mainstream sector.
The role will include supervision of members of Disability and coordination of the Health and Education Support Worker Project.
